Matrix Providers’ health care benefits provide coverage when it’s needed — with a focus on programs that support and encourage healthy living and your overall well-being.
Medical and prescription benefits
Matrix Providers’ employees have a choice of two medical plans administered by United Healthcare and Kaiser Permanente. You will have access to both a PPO plan and an HDHP plan as well as HMO. Each medical plan provides comprehensive coverage, including preventive care covered at 100% and prescription benefits, as well as an extensive provider network.
Dental
Affordable dental coverage through United Healthcare helps keep you smiling. You have a choice of two — the United Healthcare Plan Low and the United Healthcare Plan High. The plans cover preventive, basic, and major care. The United Healthcare Plan High covers orthodontia services as well.
Vision benefits
You have access to vision coverage through United Healthcare using the VSP Network that helps cover the cost of eye care, including standard vision exams, eyeglass frames and lenses, and contact lenses.

401(k) Savings Plan
The Matrix Providers 401(k) Savings Plan is funded by employee contributions and contributions from the Company. It offers a wide choice of investment options, and includes a Company matching contribution.

Opportunity to purchase additional coverage:
• Employee – up 7x your base pay or $500,000. The guarantee issue is $130,000
• Spouse – 100% of Employee coverage up to $250,000
• Children – up to $10,000 depending on child’s age

• STD – up to 60% of your weekly salary
• LTD – up to 60% of your base monthly salary
• Maximum benefit $2,000 per week $10,000 per month
